According to the American Geriatric Society, roughly 62% of all senior hospital readmissions are preventable. The top-four risks of readmission are:

- Medication errors
- Nutritional deficiencies
- Failure to follow care plans following a hospital discharge
“[People at risk for falling or who can’t manage their medication] may have trouble with wellness and nutritious meals. We wanted to figure out a way to take and broaden that continuum of care and solve those pain points for families,” said Dickison.
If, for example, one of our clients has trouble adhering to a medication regimen, they could add Direct Link® monitoring technology to their package. If a client tends to forget when to eat, drink, or exercise, we can add scheduled check-in calls to their program as a friendly reminder.
Many of our clients don’t need 24/7 in-home care. By adding wellness calls on days when Caregivers aren’t in the home, we can provide seniors with the care they need and families with peace of mind.
